Targeted Case Management (TCM) Services


    

Carobell began offering Targeted Case Management to the community in July 2007. This Service is offered to the entire OCBHS catchment area, which includes; Onslow and Carteret County.  Carobell decided to become a provider of TCM service based on the extensive need in the community to have knowledgeable, caring, and compassionate people who understand the importance of building a strong and committed relationship with the persons served. By truly knowing the Person served, the Targeted Case Manager has a strong foundation in developing the Person Centered Plan (PCP) and linking the person to those agencies who can best meet their needs.

 

    

Targeted Case Management is a service to individuals with a diagnosis of Mental Retardation / Developmental Disability, or individuals who are at risk for developmental disabilities or delays manifested before age 22. It is offered statewide and is paid for by both Medicaid and State dollars. Carobell’s TCM program primary focus is to assist individuals in gaining access to needed services in their communities that will help support and teach the individual the skills they need to be empowered in reaching their highest potential in skills development and independence.

     

Some of the primary responsibilities of our Targeted Case Management include:

  • Facilitating the service delivery process.
  • Developing a Person Centered Plan, assuring that it addresses all the needs and desired outcomes expressed by the individual.
  • Locating and coordinating sources of support whether it is a natural or community support service.
  • Continually monitoring the plan and the services identified to assure they continue to meet the individual’s needs.
  • Keep the individual and any significant others informed of changes that may affect their services.
  • To make changes and updates to the plan as needed.
