POSITION TITLE: Community Service Coordinator Job Description
DEPARTMENT: Programming
REPORTS TO: Director of Programming

PURPOSE:
Assisting and connecting consumers and families to an array of community based services and supports to promote choice, self-direction and community involvement. Identify community resources and meet all MCO’s contract requirements. Coordinate with Vocational Coordinators and external agencies in providing services.
JOB DESCRIPTION:
The Community Service Coordinator offers comprehensive care coordination and monitoring to individuals having a primary intellectual/developmental disability, or other diagnosis. The employee works under the supervision of the Programming Director. This position requires a proactive approach to assessment, monitoring and coordination of care, to ensure quality supports and consistent adherence to waiver requirements. This position works as a part of an integrated team providing intervention to ensure individuals identified receive appropriate assessment and services, and completes required documentation. Accountable for ongoing monitoring of the provision of services included in the individual’s Person Centered Plan. Spearhead the process of assessment and reassessment of the individual’s level of care and the review of Person Centered Plan.
Principle Accountabilities:
- Maintain the HIPAA confidentiality/security standards of all information transactions pertaining to the members and staff;
- Obtains input from the member/providers/significant others about the service delivery process and seeks information in an effort to obtain needed services and supports on behalf of the Value and support waiver participants to be fully functioning members of their community;
- Assists with audits, billing, claims, and all other financial matter pertaining to day support programs;
- Offers service options that will facilitate each participant’s ability to live in homes of their choice, have employment or engage in a meaningful day of their choice and achieve their life goals;
- Provides the opportunity for all participants to direct their services to the extent that they chose;
- Assists providing educational opportunities, natural support networks and enables participants to be less reliant on formal support systems;
- Locates and coordinates sources of help so that the consumer receives available natural and community
supports;
- Facilitates the service delivery process, including innovation waiver beginning with intake/initial assessment and including the identification and procurement of services, on-going monitoring of care and services, and the annual re-evaluation of the individual’s needs and services;
- Monitors the member’s consumer status and situation to assure quality care as well as the continued appropriateness of services through phone and personal contacts;
- Facilitates the Accessibility Committee, including responsibility for the annual plan; carries out duties under the directive of the Program
- Completes application forms to assist in receiving community and other formal service support and
Retrieves/monitors status of consumers, using the Service Authorization section;
- Monitors/prints consumer PCP, assessments, and authorization letters for consumer files, using the Plan
Development section.
- Checks/monitors consumer authorizations and then prints/copies/files the authorization Makes phone contact with any authorization letter problems;

- Oversees, and facilitates meetings regarding possible new consumers or potential consumers in both
Accountable for audits, record review and protective actions conducted by the state or MCO’s;
- Troubleshoots documentation problems; Responsible for data collection of information
- Accountable for preparing compliance with accreditation standards and providing resources and meeting
- time lines for accreditation process;
- Provides 24 hour, 7 days a week on-call crisis response for Community Support Services;
- Assists with the responsibility in assuring respite staff meet and maintain all requirements as listed in the provider Contract and Agreement. including all trainings;
- Accountable for completing a monthly summary of Respite services to the Director of Accounting and keeps the Program Director up to date on any changes, issues, etc. and follows up with all finance related concerns in a timely manner.
- Accountable for Respite staff time sheets, obtains reviews, completes and forwards the weekly attendance sheets to the partnership. Assists with providing initial Respite Orientation Training;
- Maintains records on each family being served and maintains community charts.
- Assists as needed with the implementation of the JCF facility as directed by the Program Director;
- Attend all trainings, meetings as designated by Programming Director;
- Works with Vocational Coordinators in the implementation of Day Support Services;
- Completes all consumer reauthorization every 6 months and for all other services as needed for quarterly notes.
- Assists with the contractual requirements for the MCO’s.
- Accountable for assisting staff with all Day Program Support Services.
- Accountable for assisting, identifying and eliminating any risks that may result in property loss, injury or legal liability through immediate action, recommendation to Programming Director, and/or participation in team based risk management approaches.
- Accountable for assisting in writing of goals and summary progress notes, maintaining grids and completing NC Snaps.
- Accountable for assisting with CARF accreditation.
- Performs other related duties as deemed necessary.
RECRUITMENT STANDARDS:
Must hold a Bachelor’s Degree in a Human Service Field and have at least one year of paid full-time experience (or its equivalent) working directly with persons with mental retardation or other developmental disabilities.
MINIMUM REQUIREMENTS:
Graduation from a four-year college or university with a Bachelor’s Degree in special education, elementary education, or early childhood education, psychology, nursing, child development, counseling, infant mental health, social work, child and family studies and two years of professional experience working with the population served; or graduation from a four-year college or university with a Bachelor’s Degree in another human service field and three years of professional experience working with the population served; or an equivalent combination of training and experience. Employee must qualify as a Qualified Professional for Developmental Disabilities.
ADDITIONAL TRAINING/EXPERIENCE:
Employee must successfully complete competency-based training in Person Centered Planning. Prefer training or experience conducting clinical assessment interviews, consultation/collateral efforts with multiple providers, DSM V diagnosis, development and implementation of Person Centered Plan/goals in the developmental administration, repairs and maintenance, in-service training, and ISP administration, files management, correspondence preparation and purchasing/procurement, disability, substance disorder, and in the mental health field.
Employee must exhibit supervisory capabilities with leadership qualities. Employee must have strong oral and written communication skills. Employee must have the ability to use tactful judgment in dealing with parents, members, staff and administration. Employee must be able to maintain accurate records; identify and report changes in members’ condition; and remain calm in an emergency.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
- Must have basic computer, and typing skills.
- Must have strong oral and written communication skills.
- Must be able to work as a team member.
- Must have knowledge of the Waiver requirements, State Service definitions, ICF State Regulations, Program department policies.
- Must have good judgment; discretion in maintaining the confidentiality of Carobell, Inc. business as well as
- member information.
- Must be able to use tactful judgment in dealing with parents, residents, staff and administration.
- Must be able to maintain accurate records.
- Must be able to work under pressure and remain calm and effective in emergency situations.
MINIMUM PHYSICAL REQUIREMENTS

- Ability to perform a combination of standing, walking, pushing, pulling, lifting, and sitting for up to 8 hours.
- Ability to bend at the waist to a minimum of 45°
- Ability to twist at the waist to a minimum 90°
- Ability to squat to knee level.
- Ability to raise one’s self, unaided, from one knee to a standing position
- Ability to lift up to 20 pounds unaided.
- Ability to lift up to 80 pounds
- Ability to maneuver individuals served from supine, sitting, reclining, etc. position to lift, wheelchair, bed, bath, etc.
- Ability to operate mechanical and electrical lifts.
- Ability to push individual served (up to 200 lbs.) in wheelchair.
- Ability to function in outside environment for up to eight hours.
- Ability to enter and exit vehicles and assist individuals served to do same.
