Cost (per pay period): Employee Only from $50 to $85; Employee + children from $400 to $480.

Cost (per pay period): Employee Only is $5.29; Employee Plus from $10-$16.
Life Insurance, Accidential Death and Dismemberment, and Long Term Disability Insurance provided at NO COST for Employee.
Dependent Life insurance coverage for children and spouse provided for $2 per pay period.
